In order to get a Colorado Medical Hops Hash Card you must have a qualifying condition. Under Colorado law some qualifying conditions include but are not limited to:

AIDS/HIV

Arthritis

Asthma

Cachexia

Chronic Pain

Degenerative Disk Disease

Epilepsy

Fibromyalgia

Glaucoma

GERD

Hepatitis C

Migraines

Multiple Sclerosis

Parkinson’s Disease

Seizures

We can help you to complete all necessary paperwork and provide you with caregiver services quickly and easily. In order to get started you should schedule and appointment with us. You must bring the following information to your appointment:

Recent chart notes from your own doctor, with documented diagnosis*

Valid Colorado driver license or ID card

You have two options when scheduling an appointment. You can fill out our online scheduling form or you can call us directly at; 719-555-1212. If you call to schedule an appointment, please leave your telephone number with area code. Please speak slowly, clearly and spell your last name. One of our representatives will call you back as soon as possible, generally within a couple days.

After our doctor has seen you and evaluated your records, he/she will determine your need for a Colorado Medical Hops Hash Card. If you qualify, he/she will sign your permit and provide you with instructions on how to register the paperwork with the Colorado Medical Hops Hash Registry (CMMR).

*Our doctors are authorized to see patients solely to determine their qualifications for receiving medical cannabis. They cannot and will not act in any other capacity. They cannot act as your primary physician.

We require all patients to have at least one other current physician. You must be able to provide current medical records from your current physician that documents the existing condition(s) for which medical hops hash can be authorized under Colorado State Law. You must obtain these records yourself and bring them into our office. You can also authorize your medical records be faxed or mailed to our office as well.

It is important to not that our doctor’s only need the chart notes from 2 or 3 visits to your current physician that pertain to your qualifying condition(s). When obtaining your current medical records please ensure that they include any relevant x-ray or MRI reports (not actual film), and also YOUR name and phone number.

Before you can use and consume medical hops hash, you first apply for a medical hops hash identification card. You should fill up all forms of the Colorado medical hops hash forms including the Application for the Identification Card and Physician Certification Form which is filled up by the physician who recommended the treatment. You can contact the Medical Hops Hash Registry if you have more inquiries. All pertinent documents and application forms should be complete otherwise it will be returned to the applicant.

Here is the step-by-step procedure for the identification card application:

1.   The Colorado medical hops hash forms must be complete upon application.

2.   You can pick a primary caregiver and put it on your application though you are not really obliged to do so. The caregiver will be the one responsible for the management of your well-being and must therefore be at least eighteen years old or older.

3.   Next is to fill up the medical doctor information.

4.   Affix your signature and the date of application.

5.   Have your physician complete and sign the Physician Certification form. Licensed medical doctor can only sign this form in Colorado.

6.   Give your complete application to the registry within the 60 days when the physician affixed his or her signature. Photocopy of valid ID specified by the registry should be presented upon application.

7.   The application fee is .00 (non-refundable).

8.   Send all your requirements to the Colorado Public Health and Environment Department.

Verification will be done within 30 days after the application has been filed. Once approved, you will be able to receive your identification card within 5 days after the verification.

Only one card will be given to each patient. The caregiver, if designated, will not be given any card. Be sure to keep a copy of the documents you submitted to the registry for future use or in case your identification card will be lost.

Currently, these states include Maine, Maryland, Montana, Nevada, Hawaii, Colorado, California, Alaska, Vermont, Rhode Island, New Mexico, Oregon, Michigan and Washington. Each state also has its own registration system with slight variations and differences with regards to the application process. Tip 1 – Know Your State LawsOne of the most important aspects of obtaining a medical hops hash card is to first do some research into your state laws regarding the application for the card. These laws are typically available online for review, often with a section of Frequently Asked Questions to help you understand the specific issues included within the statute. In addition to providing information on what you need to complete the application, the state law information will also be up to date on what the application fee is for the card.  These application or registration fees are typically less than 0, often with a lesser amount for those receiving Medicaid, SSI or SSD. Tip 2 – Talk to Your PhysicianMost states require detailed information on your medical condition, treatment options and the reason that medical hops hash is recommended on the application.
